► Download our form to fill out on your desktop and email us
(attach it to your email).

** Every Order is subject to Sales tax of 5% GST and 7.5% PST and a delivery fee.

** You can pay by Paypal, once the order is complete we can send the request to your valid paypal account for payment.

** The form requires Adobe Reader which comes with every computer but if you don't have it you can download it here (its free).
